An 84-year-old woman was admitted for a left-sided stroke (see cerebrovascular disease) manifested by expressive aphasia and right-sided weakness. The signs lasted longer than 24 hours before gradually resolving. Head MRI showed a diffuse left sided infarct. A carotid bruit was present on the left and the magnetic resonance angiogram (MRA) shown demonstrated critical narrowing (string sign) of the left internal carotid.

A close-up of the left internal carotid shows the irregular plaque. Doppler velocity was markedly increased (354 cm/sec peak systolic velocity) in the left internal carotid indicating greater than 70% (class D+) stenosis.

The patient was taken to the operating room for left carotid endarterectomy. She was positioned with the neck extended and head turned to the right. The line of incision along the anterior border of the sternocleidomastoid (see neck anatomy) is indicated.
