A double loop (surgeon’s throw) is created for the first throw by leading the suture clockwise around the closed needle driver tip while advancing the needle driver through the loops like a piston.

The execution of the surgeon’s throw is shown. The wrap is kept close to the tissue (“strafing”) to avoid upward drift. The loops are kept loose around the needle driver to avoid “strangulation” and difficulty extracting the instrument.
