The three centimeter gastric myotomy is shown with the widely mobilized muscle edges to prevent scarring and recurrence of symptoms. The procedure was completed with a partial (270 degree, Toupet) gastric wrap. The wrap was done to prevent reflux that often accompanies esophagomyotomy. A complete wrap was not done because with decreased esophageal contractility, a complete wrap might cause obstructive symptoms.
