A 58-year-old cirrhotic man presented with a two day history of incarcerated umbilical hernia, nausea and vomiting. The abdomen was distended with ascites.

The abdominal X-ray showed air fluid levels consistent with small bowel obstruction.

The skin over the hernia was reddened. The hernia was moderately tender on palpation. The remainder of the abdomen was soft and non-tender. A gentle attempt at reduction was unsuccessful.
