The mobilized esophagus within the hiatus is shown. In this case the posterior vagus was easily mobilized anteriorly with the esophagus. When the posterior vagus is more widely spaced from the esophagus, it may be left posteriorly in the V of the crura.

A 60 Fr Hurst dilator was passed into the stomach to size the hiatus appropriately. The anterior part of the hiatus was pinched together to determine the most anatomic way to narrow the opening.

The first stitch of braided 0, non-absorbable suture material was passed through the lower portion of the left crus with a ski needle.
