A 26-year-old man presented with a seizure involving left upper extremity sensation, and was worked up with MRI. Balanced, weighted axial MRI showed a large right arteriovenous malformation (AVM) located behind the central sulcus and the motor strip.

Sagittal MRI views demonstrated the right parietal AVM and enlarged pericallosal and callosalmarginal feeders.

Angiography was done to select the target for maximum effect of embolization. AP and lateral views demonstrated an intranidal aneurysm (the major feeding vessel was aneurysmal and entered the base of the AVM).
