A fascial opening is created at the base of the umbilicus more than adequate to accommodate the appendix. The appendix is then brought up through the fascial opening and the cecum anchored to the undersurface of the abdominal wall with interrupted silk sutures. The appendix is then transected in an oblique fashion such that the shortest portion of the appendix matches the apex of the umbilical skin incision.

The appendix is then anastomosed circumferentially to the V-shaped umbilical skin opening with interrupted 5-0 chromic suture.

The appendicostomy is cannulated one final time to ensure patency. The 8 French feeding tube is sutured to the skin with a 4-0 nylon suture.
