The genitofemoral neurovascular bundle was left superficial to the mesh. In some patients, the nerve penetrates transversalis fascia too medial to do this and must either be left deep to the mesh or sacrificed.

The mesh was sutured in continuous fashion with two 2-0 polypropylene sutures to the deep lamina of the anterior rectus sheath and internal oblique muscle above, and to inguinal ligament below, creating a snug fit over the direct space. The tails were tailored as the deep ring was approached, and the ends tucked inside the external oblique aponeurosis laterally. The tails were sutured together with the final stitches, and the two suture ends were tied together. The ring was tailored to a 1 cm size with a figure-of-eight suture lateral to the cord.
