A 61-year-old woman presented with a several month history of progressive right sided retro-orbital headache and diplopia, exacerbated by exercise. She had a history of hypertension. Lateral skull film showed enlargement of the sella turcica suggestive of a pituitary macroadenoma. MRI scan showed a right intracavernous mass.

CT angiogram showed the mass to be a giant aneurysm of the intracavernous right internal carotid artery, eroding into the sphenoid sinus.
