A 58-year-old man had a 4-month history of increasing transient ischemic attacks (TIA's) (see cerebrovascular disease) manifesting as right-sided hemiparesis and aphasia (there was a prior history of partial amaurosis). The patient was having several such episodes per day. The lateral angiogram of the left carotid shows total occlusion of the left internal carotid artery.

The right internal carotid (AP view) was patent with no flow between right and left circulations via an anterior communicating artery (see brain circulation.)

The vertebral injection showed that the only flow to the left-sided anterior circulation was via a narrow posterior communicating artery.
