As the demand for blood flow to the arm increases with exercise, blood is drained retrograde down the ipsilateral vertebral artery due to the lowered pressure in the arm, and vertebrobasilar neurologic symptoms result.

The correction is surgical bypass, usually from the carotid to the subclavian.
