A tear in the middle meningeal artery due to parietal skull fracture may result in an epidural hematoma. There is typically an initial loss of consciousness due to the initial concussion followed by a lucid interval. As the hematoma dissects the dura away from the skull and presses on the brain, the patient again loses consciousness. Ipsilateral pupillary dilatation is a common sign and prompt burr hole is life saving.
