The clamp is flipped back to the front (cleats up), and the stay sutures are unwound from the cleats. They may be cut or left long for manipulation if an additional stitch is needed.

Single (Kleinert) guard clamps are placed proximally and distally and the Louisville clamp is removed. The background would normally be removed at this point, but is left here for clarity.

The fat pad is placed over the anastomosis for five minutes to allow the holes to seal with fibrin.
