A pilot hole for the intramedullary guide system was created with a drill laterally on the humeral head and posterior to the bicipital groove.

The intramedullary guide system was placed for resection of the proximal humeral head.

Transfixion pins were placed into the proximal humerus to secure the cutting template guide. Anteriorly, rotation reference pins are present to assist with selection of correct retroversion.
