During the formation of the ventral body wall in the third month, scrotal/labial outpouchings appear in the inguinal region. The somatic lining of these pouches is the transversalis portion of the endoabdominal fascia. A sleeve of parietal peritoneum, the processus vaginalis, forms an inner lining.

During the fifth week, the gonads form on the anteromedial surface of the nephrogenic ridges in the retroperitoneum. Sex differentiation occurs during week 7. The gonads are anchored to the base of the scrotum/labia by a ligament (gubernaculum in the male, round ligaments in the female).
