The same three fingers of the right hand grasp the free end of the working strand, leaving the thumb and index free. The loop is kept loose and open so no tension is transmitted to the tissue.

The knot is set using the finger that is dependent to the knot, in this case the left index finger. The setting finger alternates from hand to hand with each throw. For practice leave a small open loop for each throw to develop control and show that you are not pulling up and reversing throws into half hitches. In actual surgery the loop should be evenly closed and tightened with the fingernail of the dependent finger just below the knot. The last snugging should be accomplished with a few short jiggles to overcome friction without breaking the strand at the usual place immediately adjacent to the knot.

In preparation for the second throw, the thumb of the left hand is swung under the long end and across the first throw from left to right. If the strands had started in this position, this would have been the initial move. It doesn't matter which side the ends start on if you begin with the appropriate throw.
