Osteoarthritis, or "bone producing" arthritis, causes the deposition of "new" bone around the periphery of a joint. Osteoarthritic changes are visible as joint swelling. Osteoarthritis of the metacarpotrapezial joint is a common problem in the 40-60's, especially frequent in women, and usually bilateral. The "new" bone that forms around the edge of the joint can also contribute to narrowing of the carpal canal, and may contribute to the increased presence of carpal tunnel syndrome.

The thumb is considered to be 40% of the hand, and is mainly an opposition post against which other digits pinch. Patients with metacarpotrapezial osteoarthritis commonly complain of pain when grasping objects, using the thumb to push on something, or performing actions like unscrewing the top from a bottle. The sudden pain associated with grasping may make it difficult for patients to prevent themselves from dropping objects. In some cases, pain may be present even in the absence of motion in this (basilar) thumb joint.

The normal metacarpotrapezial joint is a shallow biconcave saddle that allows a wide range of movement and enables full opposition of the thumb. Some experts believe that ligamentous laxity of this joint may lead to the development of osteoarthritis.
