Retraction of the frontal and temporal lobes reflected the internal carotid artery medially, so the aneurysm neck could be exposed and approached from laterally.

Once the neck had been carefully dissected free and defined, the clip was placed. The anterior end of the PCOM was included in the clip. Care was taken not to include the anterior choroidal artery which takes off just distal to the PCOM.
