The denuded extensor tendon of the thumb is exposed beneath the lesion. If paratenon could have been preserved, the defect could have been covered with a full thickness skin graft without a flap. However, adhesion of a full thickness graft would restrict extensor tendon gliding, so it was necessary to proceed with the flap.
