A 28-year-old man presented with left knee instability four years after a load injury with possible patellar subluxation. Physical examination showed an anterior draw sign indicative of anterior cruciate ligament (ACL) disruption. Pre-operative lateral X-ray of the knee shows no bony avulsion or fracture. Occasionally patients will sustain tibial plateau fractures or avulse the tibial spine along with the ACL injury.
