After an initial stab wound at 3:00 in the umbilical skin fold, the skin edges are held under tension with Adson forceps and the incision is completed with a sawing motion.

Using the Adson to stretch the umbilical skin over the ring finger, the assistant exposes subcutaneous bleeders for cautery (see cautery technique).
