Maintaining the position of the mesh with the left hand, the surgeon places the first staple in pectineal ligament adjacent to the iliac vessels and vas. The ligament is usually visible through the mesh, but if there is any uncertainty the edge of the mesh should be lifted to confirm the position and avoid injury to the external iliac vein.
