Maintaining the position of the mesh with the left hand, the surgeon places the first staple in pectineal ligament adjacent to the iliac vessels and vas. The ligament is usually visible through the mesh, but if there is any uncertainty the edge of the mesh should be lifted to confirm the position and avoid injury to the external iliac vein.

Still maintaining mesh position with the left hand, additional medial staples are placed as indicated in figure 8, taking care to avoid the epigastric vessels.

At this point, the stabilizing stitch in the lateral roll is cut and the remaining mesh is unfurled under the peritoneal flaps to the lateral side.
