Polypropylene mesh was sutured to inguinal ligament, anterior rectus sheath (deep layer) and internal oblique, with the two tails recreating a 1-cm deep ring.

The ilioinguinal nerve was replaced in its anatomic position with the cord, and the external oblique aponeurosis was closed loosely with continuous 4-0 absorbable suture. Scarpa's was closed with the same material and the skin was closed with clips. With 40 cc in skin and sub-Q, 5 cc beneath external oblique, 5 cc at pubic tubercle and deep ring, and 5 cc in skin at closing, the total amount of local used was about 60 cc.
