The inguinal floor of Hesselbach's triangle was inspected and found to be lax. Any disruption of the inguinal floor (weakness of the direct space, enlarged deep ring, femoral hernia) is a type III hernia in Nyhus' classification and requires formal repair (vs. a Marcy ring pexy for a type II, or a simple high ligation for a type I indirect without ring dilatation). The critical structures used for all anatomic hernia repairs are shown in the following illustrations.
