The skin edges are separated widely by moving the fingers apart, exposing the bulge of the lesion.

A 2-0 monofilament suture on a moderate size cutting needle is used to transfix the lesion.

A second deeper bite is taken with the same suture while the ends of the first bite are lifted to elevate the lesion. With the lesion now localized and fixed, the fingers can be removed. Without these steps, it can be very difficult to keep track of the position of a lesion and find it again after the skin is opened.
