The portal vein is formed behind the neck of the pancreas by the confluence of the splenic and superior mesenteric veins (see portal vein anatomy and spleen anatomy).

This patient had tumors near the confluence of splenic and hepatic veins and in the tail of the pancreas. Although the patient was asymptomatic, concern about the potential for malignant change in tumors greater than 2cm led to the decision to operate on the pancreas. The patient was injected with 4 microcuries of indium labeled octreotide 24 hours prior to surgery for intraoperative scintigraphy.
