If cephalad retraction of the fundus with a locking blunt grasper through the anterior axillary line port easily exposes the infundibulum, as shown here, the infundibulum is grasped through the para-rectus port to further expose the cystic pedicle.

There are often adhesions between the infundibulum and the adjacent structures (omentum, duodenum). The adhesions are grasped and slowly peeled down toward the cystic duct (arrow).

If the gallbladder is tense and distended, it is aspirated, then regrasped over the puncture site.
