Patient was taken to the OR where the tissue expanders were removed. Scalp flaps were elevated and reapproximated to their maximum reach. At this point, a mark was made to determine the area of maximal approximation. The excess skin was removed and the defect was sutured. Patient remained afebrile and displayed no signs of infection.

Once the scalp tissues had healed the patient came for a follow up appointment. Upon measuring of the remaining skin graft, a 4 cm reduction was noted.

Patient was brought back to the OR for further evaluation of the remaining 6 x 16 cm defect. Using the old scars located above the right and left ear, subgaleal pockets were created. A 125 mL expander was placed on the right scalp and a 250 mL expander was placed on the left scalp. During the second insertion of the tissue expanders, the decision was made to have the ports buried under the skin. The reason for this decision was twofold: (1) to decrease the risk of infection (2) to increase patient comfort by decreasing port-site skin erosion or entanglement.
