Below the termination of the posterior rectus sheath at the arcuate line, the transversalis fascia was opened and the plane between transversalis and parietal peritoneum was carefully developed.

A frame retractor was placed and the viscera, including left colon, left ureter and left gonadal vessels are retracted to the patient's right exposing the iliac vessels and L5-S1 junction.
