A 54-year-old man presented with back pain and radiculopathy unrelieved by conservative measures. MRI revealed herniated disks at L4-5 and L5-S1.

The patient first underwent posterior diskectomy and plating of the L4-5 disk space. Due to persistent symptoms, the patient was taken to the operating room for L5-S1anterior lumbar interbody fusion (ALIF).
