A 61-year-old woman was known to have polycystic kidneys from age 27. At present she was dialysis dependent with increasing abdominal complaints.

CT angiogram shows massively enlarged polycystic kidneys occupying most of the abdominal cavity. This cut at the level of the renal hila shows small renal arteries with an accessory lower pole vessel on the right originating from the aorta below the takeoff of the inferior mesenteric artery (see renal anatomy .)
