The next key technical challenge is to elevate the subcutaneous layer off the paratenon of the tendon without violating the paratenon. The paratenon is essential for the blood supply of the skin flaps and for later excursion of the tendon.

By gently lifting up on the subcutaneous tissue, a scalpel cut will allow you to find the plane between fat and paratenon.
