Low anterior resection of the rectum, total sphincter-sparing colectomy and abdominoperineal resection require division of the middle rectal vessels to the lower rectum arising from the internal iliacs. These vessels lie on the cephalad surface of the levator muscles. Care is taken to avoid injury to the adjacent ureters.
