The external oblique is opened by making a nick with a #15 blade in line with the apex of the superficial ring about 1.5 cm cephalad to the inguinal ligament. It is beneficial to inject 3-5 cc of 1% lidocaine beneath the aponeurosis before opening to anesthetize the ilioinguinal nerve and to hydrodissect the subaponeurotic plane and protect the nerve from injury.

The closed points of a Mezzenbaum scissors are slid from the fascial nick down to the superficial ring, keeping the points up against the underside of the fascia to avoid injuring the nerve. The lower blade of the scissors is then placed under the fascial edge and the fascia is opened into the superficial ring by shearing with the closely apposed blades.

The external oblique aponeurosis is opened laterally in similar fashion. The edges of the aponeurosis is grasped with hemostats and elevated anteriorly.
