The neck of the sac is transfixed with a 2-0 silk suture and the suture is tied once on one side of the sac.

The ends of the suture are passed around the other side of the sac and tied with three throws. The sac is excised with the heel of a Mayo scissors, leaving a 1/2cm stump which should retract into the deep ring.

The suture ligature of the sac is shown. As an extra precuation, with the needle still attached to one end of the suture, the needle can be passed laterally beneath the edge of the internal oblique muscle and out. It is then passed back through the edge of the muscle from outside to in and tied to the free end of the suture, lateralizing the stump of the sac under the muscle and ensuring against recurrence.
