Even if there is an obvious direct hernia, the cremaster is opened in the line of its fibers. A central nick is made and the cremaster is opened by bluntly dissecting the plane between muscle and cord structures with a Pean clamp, holding up with the spread blades, and opening the cremaster with cautery.
