A 78-year-old man had a chest X-ray in the course of an admission for intestinal obstruction. The X-ray shows a large air-fluid level behind the heart. The patient was known to have a hiatus hernia.

The chest CT shows a large sliding (type I) hiatus hernia up to the level of the pulmonary artery and a massively dilated esophageal hiatus.

The esophagogastric junction lies at the level of the pulmonary artery, indicating the phreno-esophageal ligament is disrupted and differentiating this from a type III paraesophageal hernia.
