A 64-year-old man presented with an ulcerated lesion on his left lower back. Shave biopsy by a dermatologist showed a malignant melanoma (see melanoma discussion) with a depth of 8 mm and positive deep margin. An excisional biopsy (see melanoma narrative) with a 1-cm skin margin showed an additional 1.5 mm depth of melanoma.

On presentation to the surgery service, the patient had a palpable 2.5-cm left inguinal node. The remainder of the physical exam and laboratory values were normal.
