Two years prior to the current admission, a 29-year-old man presented with a history of chronic constipation. Workup included flexible sigmoidoscopy, upper GI series, Sitzmark study and virtual colonoscopy. The sitzmark study showed retention of 6 marker rings (out of 24) (arrows) evenly distributed through the colon.

The scout film for the virtual colonoscopy showed redundant colon, particularly sigmoid.

The virtual colonoscopy reconstruction image showed the sigmoid in a twisted configuration. The patient was diagnosed with colonic inertia and started on a high fiber diet which resulted in one bowel movement per day.
