A 44-year-old man underwent laparoscopic total extraperitoneal (see preperitoneal hernia repair) repair for bilateral inguinal hernias. After exposing a direct defect on the left, a large tortuous vessel was exposed on peeling back the posterior parietal peritoneum below the inguinal floor.

The psoas muscle was exposed between the large artery and the vas, demonstrating the absence of the external iliac artery in its normal anatomic position. This suggested that the visualized vessel was a tortuous external iliac artery rather than an accessory vessel or an anomalous persistent sciatic artery.

Retroperitoneal fat was dissected off the distal iliac to follow its course.
