A 72-year-old woman underwent upper endoscopy with monitored anesthetic care (MAC). In the recovery room she experienced chest pain and desaturated. Breath sounds were diminished bilaterally and there was subcutaneous crepitance. Stat chest X-ray showed bilateral pneumothoraces and extensive subcutaneous air.
