A 39-year-old woman presented with an infiltrating squamous cell carcinoma of the left maxillary region. CT scan showed infiltration extending to the lower eyelid, but no bone erosion or invasion of the orbit or maxillary sinus. She was scheduled for wide local excision and reconstruction with a cervicofacial rotational cheek (Mustarde) flap.

The resection margins were marked with dots and the proposed flap was outlined. Note that the facial segment of the flap is higher than the superior resection margin to avoid tension on the lower lid and eversion (ectropion) and tear spillage over the cheek (epiphora).
