The patient was taken to the operating room with a diagnosis of incarcerated femoral hernia. The sac was exposed through a right inguinal incision. The patient’s head is to the right.

A close-up of the sac with dark contents is shown. The sac was opened, and necrotic, perforated small bowel was found. Attempts to mobilize the bowel through the defect were unsuccessful (see strangulated femoral hernia), so a low midline incision was made.

The small bowel is seen entering the upper end of the femoral canal. The hernia neck was enlarged from within the abdomen and the bowel reduced.
