A 49-year-old man was noted to have a 4.7-cm right adrenal mass on two-dimensional source CT images from a virtual colonoscopy. A history was obtained of a few years of hypertension, fatigue and heat intolerance. Serum metanephrine and normetanephrine levels were 5 and 10 times normal levels respectively and 24-hour urine catecholamines were similarly elevated. Serum aldosterone was normal as were thyroid function and imaging studies.

CT scan demonstrated the lesion posterior to the inferior vena cava. The lesion enhanced with contrast from 40-64 Hounsfield units.

The lesion is shown in orange with a small rim of normal adrenal (yellow). The IVC is blue and the normal left adrenal is yellow.
