The kidneys lie about the level of L1-L3, with the right slightly more caudal than the left. The slope of the underlying psoas muscles makes the lower poles farther from the spine than the upper poles. The kidneys are surrounded by a cushioning layer of fat and are enclosed in an envelope of (Gerota's) fascia.

The abdominal cavity is lined with a layer of endoabdominal fascia (commonly called transversalis fascia). The posterior lamina of Gerota's fascia lies against the transversalis fascia. A small portion of the upper pole of each kidney not covered by other viscera has a layer of parietal peritoneum in contact with the anterior lamina of Gerota's fascia. The layers of Gerota's continue across midline, fuse lateral to the kidneys (lateroconal fascia) and enclose the adrenals above and the ureters below the kidneys.

Seen from behind with transparency, the right kidney lies behind the second portion of the duodenum and pancreatic head. Note the partially retrocaval position of the right adrenal gland. The left kidney lies behind the tail of pancreas, stomach and spleen.
