A 66-year-old woman presented with reflux esophagitis. Barium study showed a sliding (type I) hiatus hernia.

CT scan shows the hernia above the diaphragm. Manometry showed normal esophageal body pressures and low lower esophageal sphincter (LES) pressure.

The patient had had multiple abdominal operations and open repair was chosen over laparoscopic for safety reasons. There were multiple adhesions between stomach, liver and anterior abdominal wall that had to be dissected to expose the hiatus. The esophagus was exposed and encircled. The anterior vagus nerve can be seen on the esophagus.
