A 40-year-old man presented with a two month history of epigastric pain, post prandial fullness with emesis of undigested food. On admission the patient was dehydrated and tolerating only liquids Chest X-ray shows a large gastric air bubble.

Upper GI series showed an apple core lesion in the third portion of the duodenum (D3).

CT scan showed the lesion just to the left of the superior mesenteric vessels. There was no celiac or periaortic adenopathy noted.
