A 34-year-old woman was worked up for a complaint of lower abdominal pain. She had a history of drainage of a perianal “cyst” several years prior to presentation. On rectal exam a soft fullness was felt posteriorly. CT scan showed a heterogeneous multilobulated retrorectal mass.
