A 22-year-old man presented with a left inguinal hernia. Skin and sub-Q were anesthetized with local infiltration of 20 cc of 1% lidocaine and 1/4% Marcaine without epinephrine (see hernia repair under local anesthesia.) The patient received light conscious sedation through the case and experienced no discomfort after the initial injection. The superficial epigastric vessels (see hernia repair under local anesthesia, frame 5) were ligated and divided. External oblique aponeurosis was exposed and the superficial inguinal ring is demonstrated (medial to the left.)

An additional 5 cc of local was infiltrated beneath external oblique (see anterior inguinal hernia anatomy) to anesthetize the ilioinguinal nerve and help protect it by hydrodissection. A nick was made in external oblique with a #15 blade in line with the apex of the superficial ring.
